pkg/proc/native,pkg/proc/amd64util: xsave decoding cleanup (#3840)
- move the cpuid querying code to pkg/proc/native/cpuid since
  pkg/proc/native is the only package entitled to calling it
- add a type to describe the xstate_bv bitmap instead of using
  hardcoded constants everywhere
- use xcr0 instead of xstate_bv for the offset heuristic like gdb does

proc: use CPUID to determine ZMM_Hi256 region offset (#3831)
The offset of state component i can be found via
CPUID.(EAX=0DH,ECX=i):EBX. The ZMM_Hi256 is state component 6, so we use
CPUID to enumerate the offset instead of hardcoding.

For core dumps, we guess the ZMM_Hi256 offset based on xcr0 and the
length of xsave region. The logic comes from binutils-gdb.

Fixes #3827.

proc/native: fix syscalls to SYS_PROCESS_VM_READV (and WRITEV) (#3273)
Per https://pkg.go.dev/unsafe#Pointer conversions from unsafe.Pointer
to uintptr are only safe in limited circumstances. In particular only
conversions made in the syscall call are pinned.
Additionally add a call to runtime.KeepAlive to mitigate the bug
described in: https://github.com/golang/go/issues/58351

*: run go1.19 'go fmt' on everything and fix problems (#3031)
Go 1.19 also formats doc comments according to the new godoc syntax.
Some of our comments, especially unexported symbols did not conform to
the godoc syntax and therefore are mangled by 'go fmt'.

This PR runs 'go fmt' from go1.19 on everything and manually fixes the
problems.

proc/native/linux: replace uses of unix.Iovec for target addresses (#2922)
Replaces sys.Iovec with a similar struct that uses uintptr instead of
*byte for the base field when referring to addresses of the target
process, so that we do not generate invalid pointers.

Fixes #2919

terminal,service,proc/*: adds dump command (gcore equivalent) (#2173)
* proc/core: off-by-one error reading ELF core files

core.(*splicedMemory).ReadMemory checked the entry interval
erroneously when dealing with contiguous entries.

* terminal,service,proc/*: adds dump command (gcore equivalent)

Adds the `dump` command that creates a core file from the target process.

Backends will need to implement a new, optional, method `MemoryMap` that
returns a list of mapped memory regions.
Additionally the method `DumpProcessNotes` can be implemented to write out
to the core file notes describing the target process and its threads. If
DumpProcessNotes is not implemented `proc.Dump` will write a description of
the process and its threads in a OS/arch-independent format (that only Delve
understands).

Currently only linux/amd64 implements `DumpProcessNotes`.

Core files are only written in ELF, there is no minidump or macho-o writers.

proc: use CPUID to determine maximum size of XSAVE area (#2256)
the maximum size of the Xsave area was previously hardcoded but the
CPUID instruction can be used to determine its maximum size.

pkg/proc,service: support linux/386 (#1884)
Implement debugging function for 386 on linux with reference to AMD64.
There are a few remaining problems that need to be solved in another time.

1. The stacktrace of cgo are not exactly as expected.
2. Not implement `core` for now.
3. Not implement `call` for now. Can't not find `runtime·debugCallV1` or
   similar function in $GOROOT/src/runtime/asm_386.s.
